This is a common issue, and it generally isn't technical.
Did you play through the game on multiplayer at any time? If you died while playing multiplayer and your friends were unable to or just didn't revive you, then beat the level without you, it doesn't count as you beating it and you'll have to replay it.
This is fairly common in the river level and catfish boss, since your friends are unable to use CPR on you if you die.

Try this: If all the levels show that you've like beaten them, beat the last boss once.
